Camper Forms

The first step towards a great summer at camp is gathering information about our campers through our camp forms.

Your cooperation in filling out and returning completed forms early will help us provide the best camp experience for your child. Each camper form is reviewed extensively by senior staff members so we can get to know your camper and his/her needs before the first day of camp.

All camper forms, including the medical forms, are required by April 15, 2010, to consider your child’s registration complete.*

*We understand that some campers may need a physical to have the medical forms completed. PLEASE submit all the other camper forms and send along the medical forms as soon as you can!
