Camper Forms

All forms are due by March 15, 2012. If you registered for camp after March 15, please submit your forms within 10 days of registering. Campers will not be able to attend camp until all completed forms are submitted. Returning completed camp forms to the camp office is one of the most important pre-camp responsibilities parents have prior to sending their children to camp. The information you provide helps us get to know your child before the first day of camp. By working together, we can assure your child a safe, happy, and memorable camp experience.

Medical Forms

All medical forms are due no later than May 1. If your child requires a physical but their birthday is after May 1, please send us the most current information now, and an updated Form 3B after their exam. No camper will be allowed at camp unless the office has received all three parts of the medical forms, completed and signed by the parent/guardian and a medical professional. We will accept school forms as long as they are signed by a licensed medical professional and cover all the information on our forms. This includes a record of all immunizations. Medical forms are reviewed by licensed camp nurses or camp administrators prior to camp.

The medical form is in three parts:

  1. Form 3A is to be completed and signed by the parent or guardian. This must be completed each year.
  2. Form 3B** must to be completed by BOTH the parent/guardian and a licensed medical professional.
  3. Form 3C is the Waiver for Administration of Medication. This form must be completed every year by both the parent/guardian and a licensed medical professional. We cannot dispense prescription or over the counter medication without a signed waiver. Therefore, you must complete this form. Keep in mind that this includes treatment for fever, bug bites, stomach aches or headaches that may arise during the day.

**If you are a returning camper, please call the office to see if we have a completed medical on file from last year. If we have a medical on file and your child’s health hasn’t changed, you will not have to have a physical or have the doctor complete this form. If we do not have a medical on file, you will need to have a physical and the medical form completed after September 2010.

All new campers must have a physical after September 2010, and a completed medical form on file before camp begins.


Friendship Requests

The process of making groups is complex and we do our best to make everyone happy. Because we know how important camp groups are to a child’s experience, we take the group placement process seriously and it is one of our biggest priorities.

Camp provides a wonderful opportunity to meet new friends. However, if you are coming to camp with a friend, you may both request to be placed in the same group.


The Code of Honor

Prior to camp, we urge parents to review the code of honor with their children. This form should be signed and returned with the other forms. These rules and policies help establish a community at camp where everyone is aware of the guidelines that support a safe and fun camping experience. We take these rules seriously at camp, and expect you and your campers’ full support.
